It's been nearly a year since the Sofa Super Store fire. Now just two days after Charleston Mayor Joe Riley (web|news|bio) said the city's internal review would be released as scheduled, the National Institute for Health and Safety has released their report.   • NIOSH gives an hour by hour account of the blaze
  • NIOSH says while crews were on the scene, they knew their were five people trapped inside
  • Also NIOSH claims that the city provided minimal training records for some of their higher ranking firefighters on the scene that day
  • Some firefighters did not have proper gear
